Required Documents for Minor's Appointment

In accordance with Texas state law, all minors under 18 must have a parent or legal guardian present at the time of the piercing and the parent or guardian must remain present during the entire appointment and will be required to complete a consent form before the procedure begins. 

 

​To verify identity and guardianship, you MUST bring the following. This helps us be fully compliant with Texas regulations and ensures a safe and verified process.

• Parent or guardian's valid photo ID (driver's license, state issued ID, or passport).

• If the child has a government-issued photo ID (such as a state ID or passport) please bring it. 

• For children who do not yet have a photo ID, an original birth certificate listing the parent/guardian's name is required. 

• If the parent's last name does not match the child's (for example, due to divorce, remarriage, or other legal name changes), please also bring documentation that confirms guardianship - such as a divorce decree, court order, or legal custody paperwork.
